Build a form and put it on your website
Create an intake, contact or sign-up form, choose what happens after someone submits, and add it to a page.
When you're done, your form is live on a page, every submission creates or updates a contact, and the person sees your thank-you message.
Before you start
- Time: 15 to 20 minutes.
- You'll need: your list of questions. Keep it short: every extra field means fewer people finish.
- Where: in the sidebar, click Forms (under Marketing). The tabs are Forms & Surveys, Templates, Submissions and Analytics.
- Choosing the type: a Form collects details (contact, intake, registration). Use a Survey for feedback, a Quiz for scored answers, and a Checkout Form to take payment.
Step 1: Create the form
- On Forms & Surveys, click Create New.
- In Choose Type, click Form.
- Or start from a ready-made one: open the Templates tab, choose Forms, and pick one close to what you need.
Step 2: Add your fields
- On the Edit tab, the Add Elements panel on the left lists fields by group: Contact Fields (First Name, Last Name, Email, Phone, Company and more), Form Fields (text, dropdowns, checkboxes, dates, file upload and more) and Display (headings and text).
- Search with Search fields..., then click or drag a field onto the form.
- Click a field on the form to select it. Its settings open on the right: set the Label, and tick Required if it must be filled in.
- Check Save Answer To on each field, so the answer lands on the right part of the contact. See Send form answers to the right contact fields.
- Use the arrows above a selected field to move it, the copy icon to duplicate it, or the trash icon to remove it.
Use Email and Phone from Contact Fields
Always use the Email and Phone fields from Contact Fields, not a plain text box. That's how Flourivate matches the submission to an existing contact instead of creating a duplicate.
If you text clients, add a consent checkbox with your exact consent wording. Only tick-box consent counts.
Step 3: Choose what happens after submit
- Click the Settings tab.
- In On Submit, set the Action:
- Show Message: shows your Success Message on the same page.
- Redirect to URL: sends them to a page you choose, like a thank-you page with your next step.
- Set the Submit Button Text, like "Send my answers" or "Get the guide".
- Form Heading / Title shows at the top of the form. Form Description is only for you; visitors don't see it.
- Scroll to Email Notifications to get an email for each submission. See See and get notified about form submissions.
- Click Save Settings.
Step 4: Publish the form
- Click Preview at the top and fill it in once to check it reads well.
- Click Save.
- Set the Draft / Publish switch at the top right to Publish.
Forms made with AI
If AI created the form, clicking Publish checks it against what you asked for. You'll see a list of differences with two choices: Fix those items or Publish anyway. Read the list. "Fix those items" is usually the right call.
Step 5: Add it to a page
- Open the page in the page builder.
- Click +, search form, and add the Form block (under Interactive).
- Click the block and choose your form in its settings.
- Save, and publish the page if it's new. See Edit and publish a page without taking it offline.
To use the form outside your website, click Integrate at the top of the form builder for its link and embed options.
Check it worked
- On the live page, in a private window, submit the form with a test email of your own.
- You see your success message or land on your redirect page.
- The submission shows on the form's Submissions tab, and the contact appears in Contacts.
If something's off
- Your settings didn't stick: changes on Settings need Save Settings.
- The form isn't on the live page: check the form is set to Publish and the page is published.
- A field moved or vanished: you may have clicked the arrows or trash icon on a selected field. Add it back from Add Elements.
- No contact was created: check the form has an Email field from Contact Fields with Save Answer To set to the contact's email.
- Want a follow-up email or tag after someone submits? Set up a workflow that starts on "Form Submitted" for this form.
