Send form answers to the right contact fields
Choose where each answer is saved, on the contact, in a custom field, or only on the submission, so your contact records stay clean.
Every question on a form, survey or funnel form has a Save Answer To setting. Set it well and each new client's record fills itself in: name, email, phone, and the answers you care about, like their goals or how they heard about you.
Before you start
- Time: 10 minutes per form.
- You'll need: the form open in the builder. If you want answers in a field of your own (like "Coaching goal" or "Referral source"), create that contact custom field first in Settings.
The four choices
| Save Answer To | What happens | Use it for |
|---|---|---|
| Contact field | Writes to a built-in field on the contact, like Email, Phone, Full Name, Company or Address. Updates the contact if they already exist, or creates a new one. | Name, email, phone, company, address |
| Contact custom field | Writes to a contact field you created | Answers you want to see on the contact and use in emails or automations |
| Response Only | Kept on the submission, not on the contact | One-off questions, like "Anything else we should know?" |
| Do Not Save | Thrown away after submit | Rare. Nothing to keep |
Steps
- Open the form and click a field.
- In its settings on the right, find Save Answer To.
- Choose one of the four options, and for a contact field or custom field, pick which one.
- Repeat for each field.
- Click Save at the top of the builder.
Built-in contact fields you can map to: Full Name (split into first and last name on the contact), First Name, Last Name, Email, Phone, Company, Job Title, Address, City, State, Postal Code, Country, Website and Source.
How matching works
When someone submits, Flourivate looks for an existing contact with the same email or phone.
- Found: that contact is updated with the new answers.
- Not found: a new contact is created.
That's why every form should have an Email field mapped to the contact's email.
Sensitive information
Intake forms sometimes ask about health or personal history. Only ask what you need. Use Response Only for answers that don't belong on the everyday contact record, and check your profession's privacy rules for what you can collect and store.
Checkout forms
Checkout forms map the same way. A buyer's first name, last name, phone and any mapped custom fields (like an SMS consent box) are saved to their contact. A consent box is only recorded as consent when it's ticked.
Check it worked
- Submit the live form with your own test details.
- Open the new contact in Contacts. Mapped answers show on the contact; Response Only answers show on the submission.
If something's off
- An answer is missing from the contact: its Save Answer To is Response Only or Do Not Save. Change it. Past submissions won't update; new ones will.
- A custom field you used was renamed or deleted: new answers fall back to the submission record, so nothing is lost. Recreate the field and remap it.
- You want one answer saved in two places: pick one target here, then copy it with an "Update field" step in a workflow.
- The phone number didn't save on a new contact: that number already belongs to another contact. Flourivate won't copy it, to avoid duplicates.
- You need the answer on a deal (opportunity): map it to the contact here, then use a workflow to copy it.
