Build a scored quiz with results

Last reviewed October 21, 2026

Create a quiz or self-assessment that scores answers and shows each person a result based on their score.

Quizzes make great lead magnets ("How burned out are you?") and learning checks for your courses. When you're done, each question carries points, the quiz adds them up, and people see a message that matches their score.

Before you start

  • Time: 30 to 45 minutes.
  • Plan your results first. Decide your score bands and what each one says, for example 0 to 40% "Running on empty", 41 to 70% "Stretched thin", 71 to 100% "Well resourced". Then write questions whose points add up to those bands.
  • Good to know: there's no quiz "mode" to pick. Results come from three things you set: each question's points, the quiz's scoring settings, and its score outcomes.

Step 1: Create the quiz

  1. Go to Forms and click Create New.
  2. In Choose Type, click Quiz.
  3. Or start from a template: on Templates, look under Education for Knowledge Assessment (scored multiple choice).

Step 2: Add questions and points

  1. On Edit, open the Quiz group in Add Elements and add a Quiz Question.
  2. Click it. Set the Label (the question) and check Question Type is Single Answer.
  3. Under Answer Options, click Add for each answer.
  4. For a right-or-wrong quiz, turn on the correct-answer switch for the right option and set Points. The question shows its points on the form, like 1 pt, and marks the correct option.
  5. Optional: write an Explanation (shown after answer) to teach as they go.
  6. Repeat for each question.

Self-assessments without a right answer

For a quiz where every answer is valid ("Rarely", "Sometimes", "Often"), give each option a calculation value and turn on Enable Calculations in Step 4, so each answer adds to the score.

Add an Email field from Contact Fields so each result is linked to a contact and you can follow up.

Step 3: Choose how questions appear

On the panel's Design tab, set Survey Type:

  • One at a Time: one question per screen. Feels lighter and works well on phones.
  • Many at a Time: all questions on one page.

This only changes how it looks, not how it scores.

Step 4: Turn on scoring

  1. Click the Settings tab and find Quiz Settings.
  2. Turn on Enable Scoring ("Calculate and display scores").
  3. Turn on Enable Calculations if you used per-option values.
  4. Optional: Randomize Questions, Randomize Options, and Time Limit (minutes) (empty means No limit).

Step 5: Write the results

  1. Still in Quiz Settings, under Score Outcomes, click Add First Outcome.
  2. Set the score percentage range and the message for that band.
  3. Click Add for the next band. Cover every percentage from 0 to 100, so nobody lands without a result.
  4. Under On Submit, choose Show Message to show the result on screen, or Redirect to URL to send people to a results page.
  5. Click Save Settings, then Save at the top.

Step 6: Test and publish

  1. Click Preview and take the quiz with answers you know the score for. Check you get the right message.
  2. Try a low, middle and high score.
  3. Set the Draft / Publish switch to Publish, and add it to a page with the Quiz block (+ > Interactive).

Check it worked

  • Each test run gives the score and message you expected.
  • Your test shows on the quiz's Submissions tab.

If something's off

  • No score shows: turn on Enable Scoring, and check questions have Points.
  • Everyone gets the same message: Score Outcomes may say No outcomes defined, or one band covers everything.
  • A question gives points for the wrong answer: check which option has the correct-answer switch on.
  • Questions appear in a strange order: Randomize Questions or Randomize Options is on.
  • You can't find your quiz: quizzes are listed with forms. Use the type filter and choose Quizzes Only.
  • A rule isn't working: rules on a question's Logic tab run top to bottom, so put the rule that should win first.
