Create a feedback survey
Ask clients for ratings and feedback, split long surveys into steps, and read the responses.
A survey is a form built for opinions: session feedback, a program check-in, or a "how likely are you to recommend me" score. When you're done, it's live and the answers collect in one place.
Before you start
- Time: 15 minutes.
- You'll need: your questions. Five to eight is plenty for most client surveys.
- Good to know: surveys use the same builder as forms. If you've built a form, this will feel familiar. See Build a form and put it on your website.
Step 1: Create the survey
- Go to Forms and click Create New.
- In Choose Type, click Survey.
- Or open the Templates tab, choose Surveys, and start from one close to what you need.
Step 2: Add questions
- On the Edit tab, use Add Elements on the left. Survey questions include Rating, NPS Score (0 to 10 "how likely are you to recommend") and Matrix (several items rated on the same scale). Regular form fields like text boxes and multiple choice work too.
- Click a question to set its wording, and tick Required where you need an answer.
- Add an Email or Full Name field from Contact Fields if you want to know who answered. Leave them out for an anonymous survey.
- Set Save Answer To on each question. Most feedback answers should be Response Only. See Send form answers to the right contact fields.
Step 3: Split it into steps (optional)
For longer surveys, drag a Step block (under Structure) between groups of questions. Each step becomes its own screen, which feels shorter to answer.
Step 4: Set the heading and button
- Click the Settings tab. Survey settings are simple: Survey Heading / Title (it starts as the survey's name). Change it to something friendly, like "How was our work together?"
- Click Save Settings.
- Back on Edit, click the Submit Button to change its Button Text.
Step 5: Publish and share
- Click Preview and take the survey once yourself.
- Click Save, then set the Draft / Publish switch to Publish.
- Share it: add it to a page with the Survey block (in the page builder, + > Interactive), or click Integrate for a link you can put in an email.
Check it worked
- Take the live survey in a private window.
- Open the survey's Submissions tab. Your response is listed under Survey Responses with its date and number of answers.
If something's off
- You can't find the survey: on Forms & Surveys, use the search box or the type filter set to Surveys Only.
- You're looking for a progress bar setting: surveys don't have one in Settings. Splitting into steps helps people see their progress.
- No names on responses: the survey has no name or email field. Add one if you need to know who answered.
- Responses aren't showing: check the survey is set to Publish and that the page it's on is published.
